Where the work gets stuck

The record exists. Finding the right version takes work.

When procedures, observations, and corrective actions live in separate files, a team spends time reconstructing what happened. Old versions can be hard to distinguish from current instructions. A historical record is more useful when people can follow the event, its owner, and the subsequent action.

We start with one real example, document the current process, and agree on a result. The proposal defines the included tools, owners, exceptions, and acceptance test. A focused First Fix covers one workflow and up to two existing applications; larger builds receive their own scope.
