Start with the decision

A report should answer a question someone can act on: which requests are blocked, which records are incomplete, or which handoffs are past the expected operating time.

We define the relevant record, the calculation, the update frequency, and what qualifies as an exception. Unsupported data should stay visibly incomplete rather than becoming a confident number.

Put ownership next to the exception

The responsible person needs the cause, source evidence, next step, and completion condition. Where a response requires approval, the system should preserve that control.

A decision log can record what was reviewed, what was assigned, and what changed. This makes follow through visible without assuming that opening a dashboard resolves the underlying problem.

Verify the measure and the workflow

Acceptance checks compare a known sample of source records with the report and test missing, outdated, or duplicated information. The team also tests whether the expected action reaches the right owner.
