THE SHORT ANSWER

The Workflow Failure Index is an Odyssey discussion framework, not a validated industry benchmark. Score a single recent workflow using observed examples. It helps prioritize questions; it does not predict savings or business performance.

Score four operating risks

For each dimension, assign 0 when the control is clear and consistently used, 1 when it works inconsistently, or 2 when it is absent or regularly fails. Record a real example beside each score.

Ownership: does a named person accept the next step? Information: does the handoff include what that person needs? Rework: does completed work return for correction? Visibility: can the owner locate the current state and the reason it is blocked?

Use the total to guide the review

Add the four scores for a total from 0 to 8. A higher total indicates more unresolved control questions in the sample, not a statistically established risk band.

Review the highest scoring dimensions first. A single serious failure can matter more than the total, especially where permissions, sensitive information, or customer commitments are involved. Do not average away a control that needs attention.

Turn the discussion into a baseline

Choose a representative period and record task volume, waiting time, active working time, correction work, and ownerless requests. Use consistent start and finish definitions.

Agree on the expected improvement and the acceptance test before implementation. Repeat the same observations after launch. The score is useful only when it leads to a concrete operating change and a measure the team can trust.
